What dermal filler does
Where the filler goes matters more than how much of it there is.
Dr Sultan places it against the bone and fat structure underneath rather than simply filling whatever looks hollow.
Is dermal filler right for you?
Filler replaces volume. It does not tighten skin, and putting more in does not make loose skin sit tighter.
It usually suits
- Cheeks, temples or the chin have flattened
- A line that is there when your face is completely still
- You want a jaw or chin defined without surgery
- You want it reversible, which hyaluronic acid filler is
It is the wrong treatment if
- Loose skin is the real problem rather than lost volume
- You are pregnant or breastfeeding

What to expect
- 01Before you arrive
- Stop acids, retinol, vitamin C and scrubs a week beforehand. If you take blood thinners, including aspirin or ibuprofen, ask your own doctor whether you can pause them.
- 02The consultation
- He goes through your medical history and what is bothering you, then photographs the area.
- 03The plan
- He tells you what the treatment will do and what it will not, before anything is injected.
- 04Numbing
- Numbing cream goes on and is left to work. Most fillers also carry their own local anaesthetic.
- 05The injections
- Needle or cannula depending on the area. He works in small amounts and checks as he goes.
- 06Straight after
- Small bumps at the injection points and some swelling. The swelling is partly the anaesthetic in the product.
- Do not touch or massage the treated area. Pressing on it can move the product.
- Keep the area clean and dry for 24 hours. Lukewarm water, air dry, no towel.
- No make-up over the injection points for 12 hours.
- No retinol, vitamin C or acids for 24 hours.
- No sauna, hot bath or jacuzzi for 24 hours.
- No exercise or anything that makes you sweat for 24 hours.
- Stay out of direct sun for 24 hours, then use a broad spectrum SPF 50 with 5 star UVA protection.

- Same day
- Swollen and possibly bruised. Bumps at the injection points settle within hours.
- 24 to 72 hours
- Most of the swelling goes. This is the stage people worry about and should not.
- 3 to 5 days
- It starts to look like the result rather than the treatment.
- 10 to 12 days
- Settled. He may offer a review at this point.
- 9 to 12 months
- The filler breaks down and is absorbed. Nothing is left behind.
What dermal filler is used for
- Lines and wrinkles
- Lines that appear when you move and lines that are there when your face is still are two different problems, and they need two different treatments.
- Facial hollowness
- Cheeks and temples that have flattened are a volume problem, not a skin problem. Tightening does nothing for it.
- Nose shape
- Filler can smooth a bump or lift a tip. It cannot make a nose smaller, and anyone telling you otherwise is selling.
